ABSTRACTPurpose: This research examined how medical staff view the Healthcare Integrated Incident Reporting System (HIIRS) concerning hospital business performance and healthcare quality improvements while understanding both barriers and facilitators in clinical environments. Theoretical Framework: The research rests on the Technology Acceptance Model (TAM) while incorporating elements from Organizational Culture Theory and the Unified Theory of Acceptance and Use of Technology (UTAUT) to examine perceived usefulness and ease of use alongside socio-organizational factors that impact technology adoption. Methodology: The study included fifty healthcare professionals in the United States who served as semi-structured interview and focus group participants. Thematic analysis was used to evaluate the data, which uncovered healthcare professionals' thoughts about integrating HIIRS, the existing barriers, and successful implementation techniques that resolved the human and cultural weaknesses of incident reporting systems. Findings: The results showed that HIIRS improves patient safety, operational efficiency, and financial performance. HIIRS adoption in one hospital reported that HIIRS reduced claims costs and legal expenses by 78% and increased patient satisfaction scores by 15%. 34% of participants reported that HIIRS was too complex and required advanced skills, which was a barrier to adoption. 67% of participants suffered from a punitive culture, which prevented them from reporting incidents. Time efficiency concerns, high implementation costs, and resistance to change were overcome with the help of the facilitators through artificial intelligence (AI) implementation, targeted training, and leadership support. Conclusions and Recommendations: Patient safety improves through HIIRS as long as the system remains usable and operates within a justice culture. However, full adoption remains limited by organizational cultural restrictions and operational barriers. The researcher proposes that healthcare organizations could implement tip sheet training programs along with cultural transformations through leadership and streamlined system processes. Another strategy is to implement AI such as large language models for analyzing incident reports, which could reduce the costs that result from incidents. The validity of HIIRS application across healthcare environments requires additional research through expanded sample sizes, longitudinal data analysis, and international exploration. It would help refine HIIRS implementations for various healthcare environments.
